This individual will be responsible for clinical Care with the hands-on healthcare coordination and oversight of resident care.
The RN (AKA Charge Nurse) will also assist with oversight of the Clinical Care Team in accordance with current Federal, State, and local regulations as well as community policies and procedures.
Job Description:
Administering medications per MAR.
Providing complete and accurate information necessary to care for the residents to the Clinical Care Assistants and individuals involved in the continuum of care.
Assisting fellow team members in maintaining a clean, homelike environment that is free from hazards by tidying up spaces, cleaning up after residents, and ensuring that they always look presentable.
Assisting in supervising the functions and training of Clinical Care Assistants through routine and unplanned meetings and education.
Ensuring that medical and nursing care is administered in accordance with the resident's wishes and per the ISP.
Monitoring that shift reports are being completed by performing rounds at the start/end of the shift.
Documenting in electronic medical record an assessment on all facility resident a minimum of once every 30 days.
Reviewing observation notes for items to be followed up on and that all tasks were completed.
Completing multiple tasks related to medications, including but not limited to; administering medications when needed, reviewing/processing all new medications, following up on missed medications, obtaining new orders for refills, participating in monthly pharmacy cycle fill, destroying medications properly, and ensuring buddy checks are being completed to prevent medication errors.
Participating in completing and reviewing all Individual Service Plans (ISPs) of residents admitted to the community and when updated.
Following-up with all incident/fall reports in the electronic medical record, notifying the Clinical Care Administrator.
Monitoring and evaluating resident care, community cleanliness, and compliance with policies and procedures by completing daily community rounds.
Interacting and assisting with resident guests and other visitors leading as an example of perfect customer service at all times.
Showing up for work on time and following the appropriate call-in procedure when necessary.
Maintaining a high standard and professional dress attire.
Following rules and regulations outlined in DHS 83 and CSL Policies and Procedures that are not specifically outlined above.
Qualifications:
Active Registered Nurse license in WI.
CBRF Certified, or ability to become, per DHS 83 regulations.
Excellent time management skills and ability to multi-task as well as prioritize work.
Excellent written and verbal communication.
Social insight and planning skills.
Proficiency in MS Office and electronic medical records.
CBRF Registry, CPR/AED certified, and Alzheimer’s/Dementia care experience preferred, but not
required.
